What items need to be included with the application?

Please include the following items with your application:

  • A copy of the house deed or Summit County Fiscal Officer’s Tax Appraisal card;
  • An itemized invoice that details the items purchased and work completed; and
  • Proof of payment (Example: paid in full receipt or canceled check).
